The information you provide is anonymous. It is optional to include your name and contact information to be entered in a raffle for a grocery gift card and/or to be put on a mailing list to receive emails about your local community health resources and ways to get involved in your community.

 The information from this survey will directly inform how Carroll Hospital, the Carroll County Health Department, and other agencies work to improve the health of our community. Carroll Hospital will create our health improvement plan based on the results. The plan will be available online for community members to view once it is completed. Previous Carroll Hospital Implementation Plans can be found on the LBH Community Health and Wellbeing page here: https://www.lifebridgehealth.org/community/community-health (see under "Listening to and Acting on Your Health Needs"). Our 2027 Plans will replace these by June 30, 2027.
